{ EXAMINATION }

June 20, 2008 - Payments for Exams due to the ACVO office.

 

July 28-29 & August 14-16, 2008 - SEE DETAILS REGARDING CHANGE IN TESTING FORMAT BELOW. Please be advised of a change with regard to the logistics of the certifying exam this summer. Briefly, the slide recognition and written portions of the exam will be administered at regional testing centers on Monday, July 28 (slide recognition), and Tuesday, July 29 (written). Candidates will be assigned one day to take the 3 practical portions on August 14, 15, or 16 at the Vet Med II Building, Room 227, University of California, Davis. These changes are also found in the examination instructions to candidates.

{ CREDENTIALS }

All Applicants for board certification by the American College of Veterinary Ophthalmologists (ACVO) must satisfy the ACVO credentialing guidelines.  The Applicant is required to complete TWO COPIES the “Applicant Form” on each occasion s/he applies to take the certification examination administered by the ACVO.  Each current Supervising Diplomate registered with the ACVO Residency Committee as mentors of the Applicant is required to complete TWO COPIES of the “Supervising Diplomate Form” on only one occasion at the completion of or within 6 months prior to completion of the Applicant’s residency. Each current Supervising Diplomate must complete a separate form for each Applicant.  The “Guidelines” document contains advice on completing these forms and a checklist for the Applicant. TWO COPIES of all completed forms and any attachments, plus the application fee must be received by the ACVO Executive Director at PO Box 1311 Meridian , Idaho 83680 , USA by 5pm (mountain time) on April 1st of the year in which the examination is to be taken.  Following satisfactory review of the attached forms, the Applicant will be provided by email (at the email address provided by the Applicant in the Applicant Form) an examination number for the ACVO certification examination. This will be emailed in early May, of the year in which the examination is to be taken.

{ ACVO RESIDENCY }

Residency applications are due a minimum of 60 days prior to the start date of a program. 

Mentor 6 month evaluation forms are due by January 31 and July 31 and resident 6 month evaluations are due by January 5 and July 5, for the respective preceding 6 month training period to Dr. Albert Mughannam (i.e. Jan-June and July-Dec). Questions, contact Dr. Albert Mughannam.
