IMPORTANT NOTICE REGARDING 2010 EXAM

The Image Recognition and Written portions of the certifying examination will be administered at regional testing centers.  The Image Recognition portion will be given on Monday, August 2, and the Written portion on Tuesday, August 3.  The practical portions of the examination will be administered at NC State University in Raleigh, NC.  Candidates will be assigned one day to take the 3 practical portions on August 11, 12, 13, or 14.  These dates are also found in the resident section of the ACVO website.  The online address to help you locate a testing center near you is: http://www.isoqualitytesting.com/locations.aspx

Certification examination instructions (2010 instructions)  

 

{ CREDENTIALS }

All Applicants for board certification by the American College of Veterinary Ophthalmologists (ACVO) must satisfy the ACVO credentialing guidelines.  The Applicant Form must be completed by all Applicants every year they apply to take the certifying examination.  The Supervising Diplomate Form must be completed by all Supervising Diplomates of Applicants who are submitting their credentials for the first time since 2007. Supervising Diplomate forms are NOT required for Applicants who successfully completed the credentials process in 2008 or after and who are reapplying to take the certifying examination. Full guidelines are provided with the downloadable Applicant and Supervising Diplomate Forms on the ACVO website. A checklist for the Applicant is also provided but need not be submitted. For further information, Applicants and Supervising Diplomates are also referred to the ACVO Credentials Committee Policies and Procedures document and the ACVO Code of Ethics. Both are available on the ACVO website.

 

APPLICATION PROCESS

 

TWO SIGNED ORIGINALS of all completed forms and any attachments, plus the application fee of $100 must be submitted to the ACVO Executive Director by US Postal Service at PO Box 1311 Meridian, Idaho 83680 or by courier to 66 N Hastings Dr, Nampa ID 83687. The office phone number is 206-466-7624.

The deadline for RECEIPT of the forms is 5pm Mountain time on January 22nd, 2010.

 

Forms received after the deadline, incomplete forms, and submissions not using original and current versions of all forms will render the Applicant’s credentials packet incomplete for that year, and the Applicant will be unable to take the certifying examination in that year.

 

EARLY DOCUMENT CHECK

 

Applications RECEIVED before 5 pm Mountain time on January 8th 2010 (2 weeks prior to the final deadline) will be checked for completeness by ACVO office staff and Applicants will be advised within 1 week of any deficiencies noted. Applicants will have the opportunity to correct these deficiencies before the deadline. This completeness check will include only the following items:

1.      That the Applicant form is present and signed

2.      That the Employment History form is present and signed

3.      That the Supervising Diplomate forms are present and signed

4.      That Supervising Diplomate forms were not mailed with the Applicant form

5.      That any attached documents are appropriately translated and certified

6.      That the Application Fee is present and appropriate

NOTE:

1.   The completeness check does not in any way judge the appropriateness or completeness of any of the information provided within the forms, nor that all or any required attachments are provided.  This will be reviewed only by the Credentials Committee following the submission deadline.

2.   

      Completeness of a packet provides absolutely no guarantee that an Applicant’s credentials will be accepted or that an examination number will be provided.

3.   Notification of deficiencies or completeness will be sent once by email to the address provided in the application. Failure to receive the email will not be grounds for appeal.  

5.   Applicants advised of deficiencies must still meet the final deadline.

{ ACVO RESIDENCY }

Residency applications are due a minimum of 60 days prior to the start date of a program. 

Mentor 6 month evaluation forms are due by January 31 and July 31 and resident 6 month evaluations are due by January 5 and July 5, for the respective preceding 6 month training period (i.e. Jan-June and July-Dec) to Dr. Ellison Bentley and Diane Hendrix at .
